Complete your details and attach documents

• Complete your personal and contact details such as address, number of your ID (passport, identity card), etc.
• Complete your educational details such as a type of diploma entitling to undertake studies, date of issue, name of your school, etc.

PLEASE NOTE that spaces marked with * are obligatory. However, it is advisable to fill in all given spaces.

• Attach copies of your diploma with supplement and other documents required at the stage of admission (qualification) for studies, together with a translation into English or Polish (in the case of study in English) or into Polish (in the case of study in Polish).
