Required documents

  • Before qualification for studies candidates are obliged to attach scanned copies of required documents.
  • Upon being qualified for studies, candidates must submit (send by post) originals/notarized copies of required documents directly to the appropriate faculty or institute.

PLEASE NOTE that non-submitting required documents by specified deadline is equal with resignation from undertaking studies. In justified cases the faculty or institute may extend the deadline for submitting documents.


REQUIRED DOCUMENTS:

  • in case of application to first (BA) or long-cycle (MA) studies – the original of secondary school (maturity) certificate and transcript of grades, entitling to undertake university-level study in country in which the certificate / document was issued
  • in case of application to second-cycle (MA) studies – the higher education diploma (MA, BA, engineer diploma or equivalent) and transcript of grades obtained during the course of study, entitling to continue university-level study in country in which the diploma was issued,
    or original of the certificate confirming graduation from studies and containing information on level, programme and profile of studies, obtained professional title and result defined in words together with statement, in which candidate undertakes to present the original or certified copy of the higher education diploma within 30 days from the day of its issuance, under pain of removal from the list of students
  • a personal questionnaire, which includes candidate’s photo, printed out from candidate’s individual registration account in the IRK system and signed by the candidate (available at the candidate’s individual registration account in section “Enrollment applications” under the chosen field of study accessible through the link “Documents and further steps”)
  • an application for issuing electronic student’s id (ELS), which includes candidate’s photo, printed out from candidate’s individual registration account in the IRK system and signed by the candidate – in case of candidates not holding ELS (available at the candidate’s individual registration account in section “Enrollment applications” under the chosen field of study accessible through the link “Documents and further steps”)
  • a proof of student’s id  (ELS) payment – in case of candidates not holding ELS [read more]
  • statement on becoming acquainted with the amount of fees for educational services at the University of Warsaw (available at the candidate’s individual registration account in section “Enrollment applications” under the chosen field of study accessible through the link “Documents and further steps”)
  • a legalisation (authorisation) or apostille (if the document was issued by an institution operating in the educational system in a country that is a party to the Convention Abolishing the Requirement of Legalisation for Foreign Public Documents, concluded in The Hague, 5 October 1961) of the original of secondary school (maturity) certificate and transcript of grades or the higher education diploma and transcript of grades – depends on the level of studies [read more]
  • original sworn translations into Polish (or English if a candidate undertakes studies conducted in English) of legalised/signed apostille secondary school (maturity) certificate and transcript of grades or the higher education diploma and transcript of grades (in case of other than sworn translation – translation legalized by the Polish consul in the country where the documents were issued) – depends on the level of studies
  • a document stating that the certificate / diploma entitles to undertake or continue university-level study in country in which the certificate / diploma was issued (with translation into the language of instruction of studies chosen by the candidate). The certificate of eligibility should be printed from the Kwalifikator system on the website of the Polish National Agency for Academic Exchange. If a given certificate/ diploma is not included in the Kwalifikator system, the document should be issued by the educational authorities of the country of issue of the document or the school/ educational institution issuing the document. Please check if your diploma entitles you to undertake first-cycle/ long-cycle/second-cycle studies in Poland (not all countries are listed in the system) here

  • if applicable, a certificate of recognition (nostrification) of secondary school certificate (issued by the appropriate Education Superintendent), in case of not holding the document – statement [a sample statement for foreigners] on obligation to submit in due date a certificate of recognition (nostrification) of secondary school (maturity) certificate, signed by the candidate [read more]
  • if your higher education diploma does not give access to further studies in a country where it was obtained – a certificate of recognition (nostrification) of higher education diploma is required, in case of not holding the document – statement on obligation to submit in due date a certificate of recognition (nostrification) of higher education diploma, signed by the candidate [read more]
  • accepted by the University of Warsaw document confirming proficiency in the Polish language (at least B2 level) – applies to studies conducted in Polish [read more]
Additionally, if applies to candidate:
  • a proof of application fee payment – in case of the need for confirmation of making the payment
  • a health certificate stating that there are no objections to undertake studies on the chosen programme – in case of organizational unit of the University of Warsaw which requires such a certificate
  • a photocopy of visa or other document proving legalisation of stay in Poland – in case of foreigners staying in Poland
  • document confirming the candidate’s right to study without tuition fee payment (for example: Card of the Pole, etc.) – in case of foreigners undertaking studies conducted in the Polish language [read more]
  • other documents required to undertake studies by the candidate in the chosen field including documents confirming grades taken into account in the qualification process as well as other documents indicated by the organizational unit of the University of Warsaw
  • authorization for submission of documents – in case of submission of documents on behalf of the candidate [a sample form – two pages]
  • Consent for undertaking studies by the under-aged applicant – declaration by the parents / legal guardians of the under-aged applicant [a sample statement in pdf format] and information clause on personal data processing which should be signed by the each of legal guardians [a sample clause in pdf format]

Please note! Documents printed out from individual registration account at IRK system must be signed by the candidate.
